Apply for a passport

The Derby Public Library has been designated an official Passport Acceptance Facility by the U.S. Department of State.

Library staff are available to accept passport applications seven days a week. 

All necessary passport application forms are available at the library, and some forms must be filled out prior to submitting the application. Those who would like to apply for a passport must provide passport photos at the time they apply.

While the library does not offer photo service, the Walgreens store in Derby (corner of Madison and K-15) does provide this service.

Passport hours:

MONDAY-THURSDAY: 9 a.m. to 8 p.m.

FRIDAY: 9 a.m. to 5:30 p.m.

SATURDAY: 9 a.m. to 4 p.m

SUNDAY: 1 to 4 p.m.

Passport services are not available when the library is closed on federal holidays or for inclement weather.

Checklists of items needed for all ages to apply for a passport

PASSPORT FEES EFFECTIVE JULY 13, 2010

Each individual passport application must be accompanied by a check or money order made payable to U.S. Department of State for all applicable fees.

ADULT (AGE 16 AND OLDER) FEES:

  • Passport book - $110
  • Passport card - $30
  • Passport book and card - $140
  • Expedite fee (optional) - $60
  • Overnight return delivery fee, available only for passport book (optional) - $15.45

MINOR (AGE 15 AND YOUNGER) FEES:

  • Passport book - $80
  • Passport card - $15
  • Passport book and card - $95
  • Expedite fee (optional) - $60
  • Overnight return delivery fee, available only for passport book (optional) - $15.45

A separate execution fee of $25 per passport application is payable to the Derby Public Library and may be paid by cash, check, money order, or credit/debit card.
